Prior to 2015 San Juan County was part of the Southeast Local Health District. In 2015 the San Juan County Local Health District was formed. Data reported are for all years using the current boundaries.
As of November 22, 2017, the IBIS system has been updated to use an algorithm which uses both ZIP and county information to designate Local Health District (LHD) and Utah Small Area. This output is based on BRFSS data collected through both landline and
cellular phones and utilizes an improved weighting methodology. For more

Denominator includes all survey respondents ages 18 years and older except those with missing, don't know and refused answers. If the query was limited to a particular sub-population-group, only those respondents are included in the denominator.
Age-adjusted rates are based on 5 age groups: 18-24, 25-34, 35-44, 45-64, and 65+, unless the query has been narrowed by the race demographic characteristic.
If race has been selected, age-adjusted rates are based on 3 age groups: 18-34, 35-49, and 50+.
When there are no observations for one or more of the age categories used for age adjustment, the response categories may not sum to 100%.
