Numerator:Resident deaths with ICD-9 codes 390-398, 402, 404, 410-429, years 1996-1998; ICD-10 codes I00-I09, I11, I13, I20-I51, after 1999, as the underlying cause of death during a calendar year.
Denominator:Total midyear resident population for the same calendar year.
